What is endometriosis?

Endometriosis is a common gynecological disease that affects almost two out of every five women unable to conceive.

It is a painful condition wherein the tissues that should be developing within the uterus grows outside instead. The ovaries, fallopian tubes, peritoneum, and intestines are the most common sites for this tissue to form.

It behaves similarly to uterine tissue during your period, breaking down and bleeding at the end of the cycle. But this blood has nowhere to go.

Due to this, the surrounding areas could become inflamed or swollen, and you may have scar tissue and lesions. They can block your fallopian

Endometriosis and infertility

The growths are not cancerous. However, if they interfere with the reproductive organs, it might be not easy to conceive:

  • Endometrial tissue that wraps around your ovaries will prevent your eggs from being released.
  • Sperm cannot travel up your fallopian tubes if the tissue is present.
  • It can also prevent a fertilized egg from making its way to your uterus through your tubes.
  • It has the potential to alter your body’s hormonal chemistry.
  • It can stimulate your immune system to attack the embryo.
  • It can impact the layer of tissue that lines your uterus where the egg implants.

Symptoms of endometriosis

Some women experience no symptoms. However, they include:

  • Menstrual irregularity
  • Discomfort with bowel movement
  • Pelvic pain
  • Infertility
  • Painful coitus

Treatment for endometriosis

There are several options available for endometriosis treatment in India at our clinic. It can be treated with hormonal medications (pills that contain progesterone) or surgically, depending on your severity and symptoms. Some treatments include:

Laparoscopic Surgery

Endometrioma, the most common form of endometriosis, occurs in the ovaries as a small balloon filled with blood and a chocolate-like brown substance.  The capsule covers the balloon.

During the procedure, Dr. Hrishikesh Pai will drain the chocolate substance and dissect the capsule from normal ovarian tissue, taking care not to damage the ovarian tissue or deplete the oocytes while removing the endometrioma.

Infertility treatment

If an endometrioma is discovered during a routine ultrasound, it may be overlooked and treated as part of a standard infertility evaluation. The doctor can scan for tubal patency and use IUI for 2 to 4 cycles of ovulation induction. If that fails, we may use IVF instead. You do not need laparoscopy before IVF.

However, in patients with large endometriomas (which can obstruct oocyte extraction during IVF) or symptoms such as discomfort, a laparoscope may be used to drain the endometrioma and remove the capsule while being careful not to harm the ovarian tissue. Carbon dioxide laser helps reduce the damage.

Alternatively, the doctor may leave the capsule parts closest to the ovarian hilum behind to minimize oocyte loss. However, the recurrence rate is higher with this partial capsule withdrawal. An AMH examination may be performed both before and after the endometrioma is removed. If AMH is reduced after surgery, you should undergo IVF treatment immediately.

Unmarried girls can undergo fertility preservation by having their oocyte retrieved and freezing them.

Reason for undergoing endometriosis treatment in India

The most important thing to remember is that if endometriosis is linked to infertility, you will need treatment to speed up pregnancy, such as IUI or IVF.It is crucial not to delay the pregnancy process unnecessarily.

When consulting with our doctor about endometriosis, it is crucial to be specific about your treatment objectives, such as improving fertility and becoming pregnant.

Some endometriosis therapies are not recommended for women who are attempting to conceive.

If you are taking hormonal medications to control the symptoms of endometriosis, you will have to discontinue them if you are planning to conceive.

Frequently Asked Questions

When should I consult the doctor?
If you are having trouble conceiving naturally and have signs that may be due to endometriosis, you should see your doctor or a fertility specialist.
How is fertility affected by endometriosis?

Severe endometriosis may deform the tubes and ovaries, and scar tissue or cysts can prevent the egg from being released. Although mild endometriosis has been linked to infertility in some cases, the exact reason is unknown.

It is also necessary to keep in mind that even women who have severe pain due to endometriosis can still conceive naturally.

Can pregnancy cure endometriosis?

Pregnancy does not cure endometriosis, as the pain can return when you get periods after giving birth.
