What is PICSI?

Physiological Intracytoplasmic Sperm Injection (PICSI) is an advanced version of the ICSI treatment that may help a subgroup of ICSI patients.

Men with inadequate sperm samples are more likely to have immature sperm. These sperm can have good motility and morphology despite being immature. They do, however, have higher quantities of damaged DNA, which can result in poor embryo quality and pregnancy loss if they are chosen for fertilization.

Instead of using visual observation (morphology and motility), which has been the only method accessible to embryologists for selecting sperm until now, we determine sperm selection using the PICSI procedure. This method helps in selection in the same way as it happens in the natural reproductive process.

What is the process of PICSI treatment in India?

Dr. Hrishikesh Pai will advise you on whether or not PICSI is suitable for you. Using sperm picked this way carries no additional concerns.

First, the sperm is deposited in a PICSI dish with hyaluronan hydrogel samples. All human cells, including the oocyte’s gel layer, contain hyaluronan, a naturally occurring biopolymer.

Only biochemically mature sperm can bind to the hydrogel, which separates them for use by the embryologist in the following step. This procedure replicates a vital phase in natural fertilization, the binding of mature sperm to the egg complex that allows only competent and developed sperm to be used in fertilization.

Due to this, the mature sperm chosen is virtually identical to the sperm that would successfully fertilize an egg in the body. Utilizing this sperm in the fertility procedures minimizes the minor variations between IVF and the normal reproductive process, allowing only the most mature and potent sperm to be selected.

The embryologist will then inject mature sperm into mature eggs. Following PICSI, the injected eggs are incubated overnight and checked for signs of fertilization the following day. The remaining of the procedure is similar to standard IVF and ICSI.

Who is the right candidate for PICSI?

The PICSI treatment in India is recommended for couples with poor sperm quality in the male partner or who have previously tried IVF with the ICSI technique and failed to obtain the desired outcomes.

The fertility specialist may recommend PCSI for the following patients:

  • Men whose sperm have a high level of DNA fragmentation.
  • Cases of recurring miscarriages with a male or unknown cause.
  • When there is poor morphology and low concentration of the sperm.
  • For patients in which the previous cycles had poor embryonic quality.

What are the advantages of PICSI treatment?

By choosing the most mature sperm with the least amount of DNA damage and a decreased likelihood of chromosomal abnormalities, the PICSI treatment becomes a very effective technique. Unlike the ICSI, which uses a subjective observation approach, the PICSI adds an objective method to the ICSI, boosting the likelihood of pregnancy while lowering the risk of miscarriage.

What is the success rate of PICSI treatment in India?

Following PICSI, the average fertilization rate equals that of ICSI and IVF, with 60-70 percent of injected eggs fertilizing normally.

Since PICSI and sperm binding depend on sperm concentration, it may not be possible for all patients who are candidates for ICSI.
